By phone:
You can call our Personalised Support Teams on the numbers below. One of our trained staff will be able to provide you with further information about our services. If you choose, a trained care co-ordinator will visit you in your home to discuss your needs and provide you with detailed information specific to your requirements. This is called our care assessment.
In person:
You can call into your local Personalised Support offices in Nottingham, Mansfield or Chesterfield to discuss your requirements. One of our trained staff will be able to provide you with further information about our services. If you choose, a trained care co-ordinator will visit you in your home to discuss your needs and provide you with detailed information specific to your requirements. This is called our care assessment. You can also have your care assessment completed in our offices.

What happens next?
If you would like to use our services, we will arrange for a trained care co-ordinator to meet with you to discuss your requirements. This is called our care assessment. At this assessment, you can expect the following:
- We will provide you with full information about our services
- We will discuss with you the range of ways we could deliver services to meet your desired lifestyle and budget
- We will provide you with information about face to face care and support, including choosing the people who might support you
- We will provide you with information about service you can use which do not require a carer visting your home, for example a panic alarm
- If you are not sure about the services you might require, we can ask you questions to help decide. These questons might include:

all aspects of health, both mental and physical, plus any concerns you have about your safety

your requirements to manage your personal care needs and domestic activities

the possibility of involvement in leisure and social activities, paid and unpaid work, learning and volunteering
- We will provide you with the full breakdown of our costs
- Will will provide you with infomation on how to pay for services and how to receive financial assistance if appropriate
- We will provide you with details of our quality assurance, so that you can see if we will provide you with a good quality service
- We will provide you with details of independent organisations who can help you with your choices
