Person Centred Planning
Person Centred Planning is central to how we plan and deliver services. It has at its core a totally inclusive approach for all customers, regardless of their abilities or differences.
We are committed to offering all of our customers support to develop a person centred plan if they don't already have one. We use a variety of methods as suited to the individual concerned and have commonly made use of Essential Lifestyle Plans, MAPs, PATHs and Portfolios. Customers can include carers, family or friends in the development of their person centred plan if they wish.
Risk assessments are carried out to ensure that decision making by customers is based upon good information and that the reduction of risk is agreed with the customer in their support plans wherever possible.
We have created a Person Centred Planning booklet to assist customers and staff to have a deeper understanding of Person Centred Plans. Our staff receive training upon joining NCHA in person centred principles and approaches, their roles and conduct and how to write a support plan. Customers are involved in the delivery of this training.
Our planning will focus on elements such as:
- Finding out what is important to you and those important in your life and what outcomes you wish to achieve
- What aspirations you have
- What skills you have and would like to build upon
- Your choices
- Regaining control
- Enablement and safe risk taking
