How Do I Apply?

Supported Living can be provided in a variety of ways with input from people specifically dedicated to ensuring your support package suits you.
Some of these people include supported living coordinators, appointees, advocates, best interest assessors to ensure the package of support meets all your individual needs.

Every person is different meaning the process can vary. The Project manager can support you to understand the process and the differing roles of the professionals involved in you circumstances.

What happens next?

Your completed application will determine the level of involvement and additional assessments required to consider suitable accommodation and your support requirements. For instance, you may wish to live in independent accommodation (single occupancy) or you may wish to share or perhaps live with more people in supported living .

If your person centred care package includes shared accommodation, then the project manager will assist in arranging introductory meetings with other individuals wishing to live in shared accommodation.

During these initial assessments funding arrangements will be discussed. Supported Living packages have hourly rates and these can be confirmed by the Project Manager. Funding information and agreements can be confusing, but we shall ensure the funding information is made available in a format best suited to you.

Following your move in we will agree a review date. This can be 4-6 weeks following your move in to see how things are going.

All of the above can be provided with timescales following the initial meeting with you. This will give you an idea of when this will happen and when you could expect to move in.
